Twenty human keratins are resolved with two-dimensional gel electrophoresis into acidic (pI 6.0) subfamilies. This antibody cocktail recognizes acidic (Type I or LMW) and basic (Type II or HMW) cytokeratins, which include CK1, CK3, CK4, CK5, CK6, CK8, CK10, CK14, CK15, CK16, and CK19. Many studies have shown the usefulness of keratins as markers in cancer research and tumor diagnosis. KRTL/KRTH is a broad spectrum anti pan-cytokeratin antibody cocktail, which differentiates epithelial tumors from non-epithelial tumors e.g. squamous vs. adenocarcinoma of the lung, liver carcinoma, breast cancer, and esophageal cancer. This MAb recognizes a protein of 56.5 kDa, identified as cytokeratin 10 (CK10). CK10 is expressed in all suprabasal layers of the epidermis. In the epidermis, expression of CK10 strictly parallels the extent of differentiation; it is absent in the basal layer, appears in the first suprabasal layers and increases in concentration towards the granular layer. However, CK10 is rarely detected in early stages of vulvar squamous carcinomas (tumors less than 2 cm, clinical stage I) regardless of the tumor grade. In larger and more advanced tumors (greater than 2 cm, clinical stages II and III), CK10 is detected very frequently. Spectrin is an actin binding protein that is a major component of the plasma membrane skeleton. Spectrins function as membrane organizers and stabilizers by forming dimers, tetramers and higher polymers. Vertebrate spectrins have two alpha-subunits (alpha-I/alpha-II), four beta-subunits (beta-I-beta-IV) and a beta-H subunit creating diversity and specialization of function. Spectrin alpha and spectrin beta are present in erythrocytes, whereas spectrin alpha II (also designated fodrin alpha) and spectrin beta I (also designated fodrin beta) are present in other somatic cells. The spectrin tetramers in erythrocytes act as barriers to lateral diffusion, but spectrin dimers seem to lack this function. Recognizes a protein of 130 kDa, identified as Desmoglein-3 (DSG3). This MAb is highly specific to Desmoglein-3 and does not cross-react with other members of the Desmoglein-family. DSG3 is a calcium-binding transmembrane glycoprotein component of desmosomes in vertebrate epithelial cells. Research has shown that DSG3 has a very high sensitivity (80%) and specificity (100%) in recognizing squamous cell carcinoma (SqCC). Therefore, DSG3 is considered a very important marker for lung SqCC and can be a useful ancillary marker to separate SqCC from other subtypes of lung cancer. This antibody reacts with the C-terminus of TGF alpha and shows no cross-reaction with EGF and the neuropeptide synenkephalin. TGF-beta (aa50) is a growth factor with 33% homology to EGF, binds to EGFR, activates tyrosine phosphorylation of the receptor, and stimulates cell proliferation. Actin is a major component of the cytoskeleton and is present in most cell types. It is highly specific to actin from smooth muscles. This MAb does not stain cardiac or skeletal muscle; however, it does stain myofibroblasts and myoepithelial cells. This antibody could be used together with anti-muscle specific actin and myogenin in making a diagnosis of smooth muscle and skeletal muscle tumors. In most cases of rhabdomyosarcoma, this antibody yields negative results whereas anti-muscle specific actin and myogenin are positive. This MAb reacts with a wide variety of simple epithelia. It does not react with stratified squamous epithelia. It reacts with epithelial tumors of the gastrointestinal tract, lung, breast, pancreas, ovary, and thyroid. Cytokeratin 18, which belongs to the type A (acidic) subfamily of low molecular weight keratins, exists in combination with cytokeratin 8. It was reported that tissues from gastrointestinal tract are positive for both cytokeratin 8 and 18 but do not contain cytokeratin 14. Perilipin-2, also known as Adipophilin, belongs to the perilipin family, members of which coat intracellular lipid storage droplets. This 48 kDa protein is associated with the lipid globule surface membrane material, and maybe involved in development and maintenance of adipose tissue. This antibody recognizes an intermediate filament protein (IFP) of 55 kDa, which is identified as cytokeratin 7. This MAb is highly specific to cytokeratin 7 and shows no cross-reaction with other IFPs. Cytokeratin 7 is a basic cytokeratin, which is found in most glandular and transitional epithelia but not in the stratified squamous epithelia. Keratin 7 is expressed in the epithelial cells of ovary, lung, and breast but not of colon, prostate, or gastrointestinal tract. Chromogranin A is present in neuroendocrine cells throughout the body, including the neuroendocrine cells of the large and small intestine, adrenal medulla and pancreatic islets. It is an excellent marker for carcinoid tumors, pheochromocytomas, paragangliomas, and other neuroendocrine tumors. Co-expression of chromogranin A and neuron specific enolase (NSE) is common in neuroendocrine neoplasms. Reportedly, co-expression of certain keratins and chromogranin indicates neuroendocrine lineage. The presence of strong anti-chromogranin staining and absence of anti-keratin staining should raise the possibility of paraganglioma. The co-expression of chromogranin and NSE is typical of neuroendocrine neoplasms. S100A2 belongs to the S100 family of proteins containing 2 EF-hand calcium-binding motifs. S100A2 may function as a modulator against excess calcium accumulation in normal human mammary epithelial cells and also have a role in suppressing tumor cell growth. This protein may have a tumor suppressor function. Chromosomal rearrangements and altered expression of this gene have been implicated in breast cancer.
