Chromogranin A is present in neuroendocrine cells throughout the body, including the neuroendocrine cells of the large and small intestine, adrenal medulla and pancreatic islets. It is an excellent marker for carcinoid tumors, pheochromocytomas, paragangliomas, and other neuroendocrine tumors. Co-expression of chromogranin A and neuron specific enolase (NSE) is common in neuroendocrine neoplasms. Reportedly, co-expression of certain keratins and chromogranin indicates neuroendocrine lineage. The presence of strong anti-chromogranin staining and absence of anti-keratin staining should raise the possibility of paraganglioma. The co-expression of chromogranin and NSE is typical of neuroendocrine neoplasms. Langerhans cells (LCs) are a subset of immature dendritic cells (DCs) that specifically localize in the epidermis and other mucosal epithelia. Epidermal LCs possess strong immuno-stimulatory capacity and play a central role in the initiation and regulation of immune responses. Langerin (CD207) is a Ca2 -dependent, C-type lectin domain containing, type II transmembrane protein that induces epidermal LCs to differentiate into Birbeck granules (BG). BGs are organelles with superimposing and zippering membranes that influence proper class I type antigen presentation to the circulating T cells. This antibody recognizes a protein doublet of 20-22 kDa, identified as MART-1 (Melanoma Antigen Recognized by T cells 1) or Melan-A. MART-1 is a newly identified melanocyte differentiation antigen recognized by autologous cytotoxic T lymphocytes. Seven other melanoma associated antigens recognized by autologous cytotoxic T cells include MAGE-1, MAGE-3, tyrosinase, gp100, gp75, BAGE-1, and GAGE-1. Subcellular fractionation shows that MART-1 is present in melanosomes and endoplasmic reticulum. This antibody labels melanomas and other tumors showing melanocytic differentiation. It is also a useful positive-marker for angiomyolipomas. This antibody recognizes CK1, CK5, CK10 and CK14. In normal epithelia, it stains stratified epithelia, myoepithelial cells and basal cells in the prostate gland and bronchi. This MAb shows no reactivity with hepatocytes, pancreatic acinar cells, proximal renal tubules, or endometrial glands; there is no reactivity with cells derived from simple epithelia. Mesenchymal tumors, lymphomas, melanomas, neural tumors, and neuroendocrine tumors are negative with this antibody. It stains myoepithelial cells and has been shown to be useful in distinguishing prostate adenocarcinoma from benign prostate. This antibody has also been useful in separating benign from malignant intraductal breast proliferations. This antibody recognizes proteins of 31-44 kDa, which are identified as Clathrin Light Chains (both A & B). Clathrin is composed of three heavy chains and three light chains, which associate non-covalently to form a triskelion structure. Clathrin light chain regulates the self-assembly of triskelions onto intracellular membranes. Clathrin light chain subunits (LCA and LCB) contribute to regulation of coated vesicle formation to sort proteins during receptor-mediated endocytosis and organelle biogenesis. LCA and LCB are encoded by two discrete genes. They share only 60% homology, and have certain features in common. Ki-67 antigen is a nuclear, non-histone protein that is present in all stages of the cell cycle except G0. This characteristic makes Ki-67 an excellent marker for proliferating cells and is commonly used as one of the prognostic factors in cancer studies. A correlation has been demonstrated between Ki-67 index and the histo-pathological grade of neoplasms. Assessment of Ki-67 expression in renal and ureter tumors shows a correlation between tumor proliferation and disease progression, thus making it possible to differentiate high-risk patients. Ki-67 expression may also prove to be important for distinguishing between malignant and benign peripheral nerve sheath tumors. Ki-67 labeling index has been shown to be a prognostic marker in a number of neoplasms including grade II astrocytoma, oligodendroglioma, colon carcinoma, and breast carcinoma. This antibody recognizes a protein of 130 kDa, identified as Desmoglein-3 (DSG3). This MAb is highly specific to Desmoglein-3 and does not cross-react with other members of the Desmoglein-family. DSG3 is a calcium-binding transmembrane glycoprotein component of desmosomes in vertebrate epithelial cells. Research has shown that DSG3 has a very high sensitivity (>80%) and specificity (100%) in recognizing squamous cell carcinoma (SqCC). Therefore, DSG3 is considered a very important marker for lung SqCC and can be a useful ancillary marker to separate SqCC from other subtypes of lung cancer. CA19-9, a carbohydrate epitope expressed on a high MW (>400 kDa) mucin glycoprotein, is a sialyl Lewisa structure which is synthesized from type 1 blood group precursor chains and is present in individuals expressing the Lewisa and/or Lewisb blood group antigens. In normal tissues, sialyl Lewisa antigen is present in ductal epithelium of the breast, kidney, salivary gland, and sweat glands. Its expression is greatly enhanced in serum as well as in the majority of tumor cells in gastrointestinal (GI) carcinomas, including adenocarcinomas of the stomach, intestine, and pancreas. Preoperative elevated CA19-9 levels in patients with stage I pancreatic carcinoma decrease to normal values following surgery. This antibody recognizes a protein of 30-33 kDa, which is identified as CD20. It is a non-Ig differentiation antigen of B-cells and its expression is restricted to normal and neoplastic B-cells, being absent from all other leukocytes and tissues. CD20 is expressed by pre B-cells and persists during all stages of B-cell maturation but is lost upon terminal differentiation into plasma cells. This MAb can be used for immunophenotyping of leukemia and malignant cells, B lymphocyte detection in peripheral blood and B cell localization in tissues. It reacts with the majority of B-cells present in peripheral blood and lymphoid tissues and their derived lymphomas. The protein encoded by this gene is a homeobox-containing developmental regulator associated with liver development. The encoded protein binds to the alpha-fetoprotein (AFP) gene promoter and increases the expression of AFP. This gene is overexpressed in some lung cancers and is linked to poor patient survival, possibly due to its resistance to cisplatin. This gene is aberrantly methylated in pancreatic cancer, deleted in squamous cell lung carcinomas, and acts as a tumor suppressor in esophageal cancer. Recognizes a protein of about 37 kDa, which is identified as Erythropoietin (EPO). Erythropoietin is a secreted, glycosylated cytokine hormone composed of four alpha helical bundles. It is the primary factor responsible for regulating erythropoiesis during steady-state conditions and in response to blood loss and hemorrhage in the adult organism. Erythropoietin is synthesized by the kidney and stimulates the proliferation and maturation of bone marrow erythroid precursor cells. Recognizes a protein of 75 kDa, identified as mu heavy chain of human immunoglobulins. It does not cross-react with alpha (IgA), gamma (IgG), epsilon (IgE), or delta (IgD), heavy chains, T-cells, monocytes, granulocytes, or erythrocytes. This MAb is useful in the identification of leukemias, plasmacytomas, and certain non-Hodgkin's lymphomas. The most common feature of these malignancies is the restricted expression of a single heavy chain class.
