Pemetrexed disodium hemipenta hydrate is a CNS-penetrant antifolate. Its pentaglutamate form inhibits thymidylate synthase (TS) with a Ki value of 1.3 nM, dihydrofolate reductase (DHFR) with a Ki value of 7.2 nM, and glycinamide ribonucleotide formyltransferase (GARFT) with a Ki value of 65 nM. Its antitumor activity may stem from simultaneous and multiple inhibition of several key folate-requiring enzymes via its polyglutamated metabolites. It is a novel classical antifolate.
