Synonym: Parathormone, Parathyrin. Summary: hPTH increases serum calcium concentrations by its actions on kidney and bone. It indirectly affects intestinal absorption of Ca++ by stimulating renal 1-hydroxylation of 25(OH)D3. PTH release is controlled in a negative feedback loop by serum concentration of Ca++. Intact PTH is the main bioactive product and is proteolytically degraded to form biologically active N-terminal fragments and inactive C-terminal fragments. C-terminal fragments are cleared only by glomerular filtration, while the bioactive intact PTH and N-terminal fragments are also degraded in the liver and other tissues. C-terminal fragment half-life increases dramatically in patients with renal failure. Thus, intact PTH level correlates best with the hormone production and biological activity.
