  • With barcoded blood bank ID numbers (BBIDs) for easy scanning into Laboratory Information System (LIS)
  • Inserts can be used for handwriting or with patient labels
  • Forms have 7-digit BBID in GS1-128 barcode format

  • General form provides two patient inserts and 18 barcoded specimen tube stickers
  • Condensed form is used for facilities that generate specimen tube stickers as needed; five stickers provided
