Plasminogen activator inhibitor 1 (PAI-1) is a secreted glycoprotein of approximately 50kDa, which functions as the primary regulator of uPA and tPA activity. PAI-1 and the related inhibitor, PAI-2, are members of the serine proteinase inhibitor (serpin) gene superfamily. PAI-1 is produced by a variety of cell types and is present in normal human plasma. It exists as two conformationally distinct forms, latent and active. The active form binds to uPA in a 1:1 complex resulting in the inhibition of proteolytic activity. Active PAI-1 rapidly converts to the latent form but can be stabilized in the active conformation by binding to the cell adhesion glycoprotein, vitronectin. Latent PAI-1 can be activated in vitro by exposure to denaturants such as SDS or guanidine. PAI-1 levels are increased in a number of disease states including coronary artery disease, venous thrombosis, obesity and various types of cancer. Recent studies indicate that elevated PAI-1 may be a useful prognostic marker for several types of cancers, particularly breast and lung.
